Accessing The NDIS

To Access The NDIS, An Individual Must Meet Certain Requirements.

This includes the Scheme being available in their area, their age, their residency status and the nature of their disability.

Am I eligible?

01. Access Request Form

The NDIS Access Request Form is now available to download.

If you think you might be eligible for the NDIS, you have several options for submitting an Access Request Form (ARF) and Supporting Evidence Form (SEF). You can call 1800 800 110 to make a verbal access request, or download the forms on your computer and email them to the NDIA with supporting information. You can also print the forms out and mail them with supporting information, or contact your local NDIA office and ask for an ARF to be mailed to you. If you would like assistance with the application process, you can contact their National Contact Centre or a local NDIS partner.

NDIS is currently testing a new computer system and some improvements to the way they deliver the NDIS in Tasmania, starting from 14 November 2022. If you have started an application in Tasmania, you will be referred to your local NDIS partner who can help you gather the necessary information and evidence to support your application.

This test is for Tasmania only and not everyone will be involved. Learn more about the Tasmania test.
