What Is Support Coordination?

What Support Coordination Services We Provide?

When you have a NDIS plan, you can decide which service providers you work with. Some people like to coordinate their service themselves, but at times it can be quiet difficult, this is when Support Coordination might be included in your NDIS plan.

There are three levels of support coordination that can be included in your plan. These are support connection, coordination of supports, and specialist support coordination.

At Diversity Camps, our support coordinators help you make the most of your NDIS funds supporting you to find the services of your choice.

At DC, we provide Level 2 Support Coordination (coordination of supports) and Independent Living options – Exploration and Design.

You need to have funding for this included in your NDIS plan, to access these services. If you don’t have this funding in your plan, give us a call and we will discuss how we till might be able to assist you.

What Can Support Coordinators Help Me With?

Support coordinators can assist you with a range of tasks related to managing your disability support services. Some of the ways in which we can help you include:

Developing a plan: Support coordinators can work with you to develop a plan that outlines your goals and needs, and identifies the support services that will help you achieve these goals.
Finding and coordinating services: Support coordinators can help you find and coordinate services such as accommodation, employment, and health services.
Advocating for your needs: Support coordinators can advocate for your needs and work to ensure that you receive the services and support that you require,
Managing your budget: Support coordinators can help you manage your funding and ensure that you are getting the most out of your allocated funds.
Monitoring your progress: Support coordinators can monitor your progress and adjust your plan as needed to ensure that you are achieving your goals.

Coordinator’s can help you navigate the complex disability support system and ensure that you are receiving the services and support that your require to live your life to the fullest.

Are DC Support Coordinators Independent?

Yes, all DC coordinators are independent. Their job is to make sure they find the best options for you by looking at all the choices possible for you and your needs.
